Bind the public to the code for digital forums

In order to give us the right to moderate comments (wherever they are given) the Code needs to be incorporated into our Terms up front on all of our websites.

Hm, I'm now realizing technically it would have to be part of Contributing guidelines on GitHub too...

If this code applies to public spaces, we need to also give them a way of reporting violations. Right now, the README asks for people to report to their supervisor at 18F.

@andrewmaier Ok - but that seems like a distinct issue. Can you open a new issue?

It says "If you are not a member of the 18F team, any member of the team can put you in touch with any of the above points of contact."
