Fix Conviction Voting Discord bot

The discord bot that displays when proposals are being created and executed is not showing the proposal title right: \n We need to update the bot to fetch data from the Gardens subgraph instead of the old Honey-pot subgraph that is deprecated. \n \n Here is an associated issue: 1Hive/conviction-voting-discord-bot#1
