Role Administrator does not exist

I have done all the steps in the readme.txt but I have a problem. When I run the application and go to create an admin an error comes up stating "Role Administrator does not exist" even tho I have added the Administrator and Doctor in the database.
Anyone that can help?

You don't need to add new fields in that table , only add Administrator and Doctor under the Name.

It is probably something really simple, but I can't figure it out. Can you showcase it with a screenshot?
