After starting the app, I cannot login / create an account, regardless of the auth type selected.

I have a similar issue. (on android phone) Once clicking on either login with facebook, twitter or microsoft I receive the message "The webpage on https://mydriving.azurewebsites.net/.auth/login/microsoft cannot be loaded due to: net::ERR_NAME_NOT_RESOLVED or net::ERR_CONNECTION_RESET

Same issue here, seems like the azure app is down.

You can consider to bypass the login. I did this by downloading the sourcecode and comment out the line which triggers the (i.e. facebook login procedure). Just return true..
