to do: add function to approve/deny ingest based on FM metadata output

dave suggested that users would want to be able to look at the output of the filemaker metadata query to check that the file being ingested is copacetic.

I had tried this with another HTML form/radio button but there needs to be some way to transfer the file paths from one php file upload process (which creates a tmp file in var/tmp before upload) to the next "accepted" upload & move to real directory.
