Title bar should display current server name with app name

Today the title bar only displays the server name. It should also display the app name for clarity eg: Developer Sanctuary - Oxidized instead of just Developer Sanctuary. Also currently the server name doesn't appear in Alt+Tab because Oxidized doesn't set the actual title of the window and just the text of the text block in the extended title bar. Since Oxidized supports multiple windows, it should set it so you know which window is for what if you've got more than one. I might even suggest adding the channel name for the window title so more clear if you have multiple Oxidized windows with the same server but different channels eg: General - UWP Community - Oxidized.
One could have the same channel in the same server on two different windows but since that's not really common, showing the channel name and server name will remove ambiguity for most scenarios.
