Held items disappear

Very rarely happens but sometimes items being held by pokemon such as eviolite/mega stone will just randomly disappear

Did the Pokémon holding the item know Bug Bite or Pluck by any chance? That move effect is bugged and will sometimes remove the wrong mon's item.

Had an eviolite on a charjabug so that was prob the case intially but then my mega stone on my Alakazam randomly disappeared like 2 badges later
