• The use of the terms "FACILITY" is eronous as NPI's are Type I and Type-2 according to the NPI final rule.. Often a Type-II does correspond to a facility but not always. For this reason, I reccomend using the codes "NPI-1" and "NPI-2", but use Individual, Facility, and Organization as suitable lables.
• Since there is only one address, it should be qualified as to what kind. It should be in the documentation and code it as PRACTICE-LOCATION. Consider leaving address off altogether for individuals but leave it on the TYPE/FACILITY ertiries.
• Type-IIs needs a name section I use the field name "organization_name".
