Update `IBMQBackend` syntax in Qermit manual

Post pytket-qiskit 0.40.0 the IBMQBackend takes a single instance string of the form instance=f"{hub}/{group}/{project}" instead of separate args for hub, group and project. This matches the syntax from the qiskit-ibm-provider.
The IBM Lagos backend is used in the manual as well which is now retired.
