Unable to upload a xlsform when creating a project

Steps to reproduce the error

1. Create a project in any org (with less than 10 members) uploading a questionnaire
2. Go trough the project creation wizard
3. After step 3 go to overview page and see that there is no xlsform

XLSForm file used is pretty normal (no weird relevant clauses, no long land_type choices, etc).
