Training Form Enhancements (Design Issues, Dropdown not working properly on Identification page)

In addition to adding the landing page for the training form and the submit button, the following bugs/enhancements should be fixed:

• General design, many pages are cramped (I'm terrible at CSS lol) and there are some fonts that are too small and other that may be too and the positioning might be off in some places too.
• On the Identification page, all of the react-native-select pickers will not update a changed selection after you select an option the first time. This is probably small bug but I couldn't find it so would love a quick hand on this one too :)
