Creating new tiles

I saw a similar type of issue on here but the solution doesn't work as I am using the defaults.
I've set up on img2img as below;

Yet every time I generate it seems to create a checker board of completely new images despite the settings being as default and using linear type. Nothing else is enable. Any idea what is going on here?

It's too big denoise. Set 0.3-0.45 or you can use controlnet (https://github.com/Coyote-A/ultimate-upscale-for-automatic1111/wiki/FAQ#how-to-use-with-controlnet)
