Add Ultrasound Cost field to Abortion Information

• What are we trying to do?

A fund asked us to start tracking ultrasound cash information on our patient records. This is spiritually pretty similar to the Abortion Cost field, so let's add it to that. And we can put it right below Abortion Cost it in Abortion Information:

• What feature or behavior is this required for?
  Keeping tabs on extra info

• How could we solve this issue? (Not knowing is okay!)
  I think roughly we need to do this:

• Adding it to the database

• Adding it to i18n

• Adding it to the view and updating the system tests

• Adding it to the export data and updating the system tests

• Anything else?
