You should just be able to add an element to the .dnn manifest to have the platform create the page for you. See dnnsoftware/Dnn.Platform@50b0238#diff-4952111b937cd772e4ce7040f3e29147 as an example (I'm not 100% sure how host pages interact with the Persona Bar)
