Header and tab bar don't remain fixed when scrolling in Form

What happens is when you click on a form element and then try to scroll, both the header and tab bar scroll along with the content window.

Reproducible on an iPhone.

Looks like this is probably an issue with iScroll 4. I noticed that someone else posted the same issue on the iScroll 4 repository.
