100% hw error

using two 5850 with v2 return 100% hw error. at @250~300Mhash/s each

Sounds like a driver bug. Try reproducing this in cgminer.

already tried a clean install with driver sweeper.

there are another prerequisite apart from clean driver?

None. If you had done something wrong, it wouldn't have started up at all.
