Improve Harvesting Reports

Increase usability of the harvesting reports to help the harvester user understand failure in the process. The harvest report needs to distinguish:
- collecting source documents
- any processing that happens
- failures in publishing to the supported output brokers
Regarding publication failures, the different output brokers may have different responses and this needs to be accounted for in the report.
