Floppy is not bootable some times

hi, i just used this utility to copy image file (w101.img , Windows 1.01 Boot Floppy running MS-DOS 3.31) , i tested this utility with other image files, floppy boots fine it the boot files are IO.SYS and MSDOS.SYS
this version of MS-DOS has IBMIO.COM and IBMDOS.COM instead, and it stucks at black screen while booting from floppy
