"Open FHIR Documentation" should work for other versions of FHIR

Currently, the extension always tries to launch documentation based on the R4 docs. Instead, it should attempt to discover a sushi-config.yaml file, and if found, it should use its fhirVersion to link to the proper set of FHIR documentation.
