Change `UninstallDisplayIcon` to `{app}\EXE\{#MyAppExeName}`

Currently the specified icon is set to one from the source directory. However, this will cause the icon to not be displayed as this directory is not available for the user. Since all icons are now available, the ChangesAssociations=yes line may be removed, too.
