Expected Behavior
System Account - Entity information page should not have any AMP Errors. Form fields should not have invalid or duplicate labels or ids.
Assumption: A form fields need to have a valid label or ids and it should not be duplicate.
Actual Behavior
AMP is showing violations for Invalid form field names and invalid or duplicate ids.
