Optimise issue layout and wording

Visually, I think the issue layout could be more clearly structured.

Maybe something like this:

[Issue: heading less than 10 words, '... should/must ...'] 'Form fields should have a label.'

[Examples] ...

[Reasoning (imperative mood, goal-oriented)] 'Help make forms accessible by ensuring that each field is correctly labeled.'

[Find out more] ...

I think that reasoning should be more prominent. Suggest moving it above examples.
