Radio and Checkbox: Ability to reveal elements

Acceptance criteria
- Content (text, other input controls, etc...) can be added between radio or checkbox items (but not attached to them, so they can be show/hidden based on other input events).
Design Sample
When option 1: show additional fields between options 1 and 2.
When option 2: show additional fields under option 2.
