QA: Document upload failed without notification

I uploaded two Word docs, intitally. They appeared to save and I was able to continue. When I went back to review my submitted report, both were marked with the message "Queueing failed."
Also, though I had already submitted my report to the approver, I was able to add new documents. (This time I chose PDFs and they appear OK.)
