Graph: Time to Follow-up: Incorrect legend categories

Should be:
Within 48 hours = Green
3 to 7 days = Orange
More than 1 week = Red
Not followed up = Grey
These are the buckets used in the dashboard example provided from 2015. Also, as in the previous issue, the data buckets and graphs need to adjust automatically to the Admin settings.
