Improve stock movements report

Based on feedback from one of the production sites:

The stock movement pdf report out of Pharmacy Usage SM.10.148856 shows the item and Quantity but not the unit cost nor the total value. The total cost is displayed in the BHIMA version on the screen but does not show the unit cost on the screen nor does it show the unit cost in the pdf report. Having this additional information in the stock movement report for items that move out of a depot would be helpful.

This per-unit information is available on the depot-to-depot stock movements, but not the stock exits to services. Update the stock movement reports for depot-to-service to add this information. At the same time, review the other stock movement reports to see if they need updating.

When doing this fix, please check #6143 for other issues that could be fixed at the same time.
