Add to Dashboard Form: Improve Usability

  1. The Dashlet title should by default be populated with the view (applicable if the form was launched from the "Share" Dropdown)

  2. The Checkbox "New Dashboard" and Dashboard should be merged into a single dropdown. It's default value would "Create new Dashboard" and the other values would be the Dashboards to choose from.

  3. The Form would be better displayed in a modal
