Statement Taking - New Feature

The ability to take statements through the CAD. Initially the idea was to do it through arrest reports directly, but I believe a better implementation would be as a feature on its own that you'd be able to attach to any arrest report. It would allow Officers to take statements when there isn't someone in custody at present but may be in the future, or just to simply have the incident recorded.
Statements are an important part of criminal proceedings and honestly right now are probably underused by Officers as there's no clean way to do it.
Please see the mock up below:
Citizen Name - Links to person's profile as usual.
Date of Incident - Selection between 'Current Date', where this then defaults the date recorded as present day, obviously, or select the select date option and the ability to use the calendar opens up.
Statement Type - Drop down box with the selection of 'Victim', 'Witness' and 'Significant Witness'. The information 'i' of this should be able to be hovered over to reveal the following helpful text;
"Victim - A person who has suffered harm, including physical, mental, emotional, or economical, which was directly caused by a criminal offence.
Witness - Someone who has seen details of an incident, whether it be partial or in full.
Significant Witness - A witness, who's statement includes details considered important, significant or pivotal for any investigation."
Title of Incident - Text box. With the example text shown until the Officer writes in it. "e.g Armed Robbery, Motor Vehicle Collision, Anonymous Threats etc."
Location of Incident - Text box. Again, example text shown until overwritten "Carson Ave, Legion Square, Diamond Casino, Los Santos Freeway Northbound etc."
Declaration - Exactly the same as the search report, just different text which I shall provide below.
Citizen declaration, recite to citizen for agreement:
"You provide this statement to the Los Santos Police Department free of duress or influence and this statement is true to the best of your knowledge. You make it knowing that you may be liable to prosecution if you have wilfully stated in it anything which you know to be false, inaccurate or misleading."
Statement Box - Finally, just as in the arrest reports, a full text both to write the statement. Should say "Statement" at the top rather than Additional Notes.

This feature would require a small addition to the arrest reports form. See mock below.
You should be able to add statements to an arrest report. Pressing the + button should open a list of arrest reports, with the most recently submitted at the top. You then have an option of deleting the statement from the report, or an eye icon to view it somehow. Possibly a window pops up to overlay the current screen - could mock a visual for this up if it would be beneficial.
