Observed with version 1.2.4 and webtrees 2.1.5
This error can occure if the module folder is renamed from repository_hierarchy to another folder name and in parallel one of the settings "Settings for source citations" is selected.
As a quick workaround to fix the issue, the module folder can be renamed to: _repository_hierarchy_

This issue can occure, if the development code (ZIP file from: Github /Repository => Code => Download ZIP) is installed instead of the release code (ZIP file from: Github /Repository => Releases => Assets)
