Just installed this with gravity forms and followed the instruction, doesn't seem to work. I see it is a little old, any idea how I could get it functioning again?

Actually is does work but I am trying to have a Move to: address and then a Move from: address. It works for Move to: but not for Move from: so it is possible to have 2 address fields and have it work for both?
