Make v0.99 release, including PDF

The PDF from this version is what we'll give to the SAC. The release itself won't contain the PDF, though, unless I set up auto PDF generation in an orphan pdf branch - so that's what I'll do. (I should have done this ages ago anyway).

Thanks very much for all your efforts getting your science case conclusions, and your final tweaks, in. The SAC are going to take a look at the paper, and (hopefully) give feedback; @ivezic is also going to seek LSST Pub Board review of Chapters 1, 2 and 12 (that had significant project input). We can make minor changes during this review, as long as we do it transparently (ie by pull request, and approval). I'll increase the protection of the master branch as high as I can to help with this. You should also take this opportunity to read Chapters 1 and 12: there is new text from the project in the introduction, including a timeline for the next few years of OpSim development. And then, watch out for comments from our reviewers!
