Do not allow submitting when any of the forms is not completed

Right now, our forms are initialized to a hidden option not_selected to prevent bias towards any of the choices.
However, we want to make it mandatory to select an option.

When the user presses submit but there are still forms without an option selected, there should be a reminder to complete all the forms.
