This form needs to subscribe a user if they don't exist, and edit their preferences if they do exist.

This one now edits the user's preferences if they exist. It also keeps in sync if they update them some other way, ie through MailChimp directly.

This now creates a new user if they don't exist. It also has an easy setting for double opt in that we can change if we need to.
