Entire fight is as normal except on 60% HP DoG just disappears!

Could you give some more information? Does he just vanish from the screen, or does he despawn?

It looks like DoG just explodes, like The Destroyer but then phase 2 doesn't start.

I fixed this by just downgrading to an older version, killing DoG, then upgrading to the latest version.

I have just come across the same problem. When the Storm Weaver was summoned by the DoG, the SW suddenly died where she spawned(it wasn't me that killed her). After that, the DoG just didn't come out of the portal.
