API needs to send name of the person completing the form field

On the LUP disposition record, The "Name of Person Completing this Form" field should get filled in upon submission. Previously we sent the string "ZAP Portal". This allows staff to query to see how many recommendations were submitted through the portal vs over email.

Remaining issues:

  1. Name of the person completing the form field

  2. When there are multiple BP's/CB's/BB's on the LUP team, each member should see their own hearing/recommendation
