fix travis and appveyor stalling issues

• appveyor builds pass the tests but then simply hang, timing out the build. See here to RDP into the worker machine on appveyor to debug.
• coverage on travis and in docker simply hangs during testing. revert commit 26b813a when fixed.

both of these may be the result of unintentionally running parallel code in tests.
