Audio and Image Embeds fail on save

Repro Steps

  1. Create a new Image embed
  2. Click Save
  3. Attempt to create a new Custom Text Embed

Expected Functionality

Both a new image embed, and a new custom text embed are created

Actual Functionality

Neither embed is created. The same functionality occurs with Audio embeds. After the failure, no other embed works.

just for the record, this was also an issue with external link (because of the thumbnail image)
