[Feature]: Link to Discord from the Help menu

Describe the Problem
Discord is one of the primary ways for people to discuss and get help with MapTool. There is even an error message for invalid assets that tells people to report the issue on Discord. But, unlike the forums, website, and wiki, there is no direct link to Discord in the app.
The Solution you'd like
A new Discord menu entry would be added to the Help menu. When clicked, this entry will open https://rptools.net/discord.
Alternatives that you've considered.

Additional Context
Using https://rptools.net/discord instead of any particular invite link will mean we are able to update the link behind the scenes in case a given invite has to be retired for whatever reason. It also means we can point it to different channels if another one becomes a better landing page for the server.
While we're at it, might as well update the Discord links in the README and other docs to be the same.
