Rework docs for individual entry pages

I'm not sure I'll be able to fix #110 without reworking the docs so it's not a single huge HTML document.

Also, splitting up the entries into separate pages while keeping the table of contents sidebar would mean that navigating to a given entry would hardly be more difficult. My experience is always: use ctrl + F to find the utility, see it in the sidebar, click it. Same thing would happen if entries were on separate pages.

So an alternate plan would be to give each entry its own page. Breadcrumb at the top (e.g. layout / ratio-box).
