Remove textarea fields from duplication request modal

Over time we have noticed that the textarea inputs in the duplication request modal get used for all sort of unintended purposes (and rarely for the intended purposes). These unintended uses can result in substantial administrative overhead to move relevant information into the correct communication channel and assure that work is appropriately assigned.

Describe the solution you'd like

Remove the textarea fields ("Description of Materials" and "Message for RAC Staff").

Additional context

If necessary, add an application config to allow these fields to be displayed in other implementations.
