Composed styles don't work

Markdown like this comes out wrong:

__*hello* world__

Here the inlineStyleRanges should range from 0 to 5 and 0 to 11 respectively.

Instead, the bold style range comes out as 0-11 and the italic style length stays at 0 o_O
