missing serial commas

I fixed one in #4
but rather than just submit more PRs (having not gotten a reply there yet), I'm opening this to point out cases of missing serial commas for consideration:
- under "Basic Concepts": "Drivers, Value and Waste" and "Domains, Delegation and Accountability" both seem to need the serial comma.
Without the serial comma, it can be read like "Drivers: Value and Waste" and "Domains: Delegation and Accountability". I don't think that's the intention (but if it is, the colon would be better).
Another case:
"The Principle of Consent: Raise, seek out and resolve objections to decisions and actions."
The serial comma would go after "seek out,"
However, I'd suggest an easier to parse definition:
"The Principle of Consent: Raise and seek out objections to decisions and actions. Then, work to resolve them."
or similar. That is longer but easier to read and to follow as a pattern.
