Handle errors with stacktrace middleware

fireplace opens this in quickfix, but doesn't go into great depths of detail.

The benefit of CIDER.el's custom syntax buffer was that it allows them to fade out tooling lines in the stack. I doubt quickfix offers anything like that. It would be good to provide a default filter of tags perhaps.

Also of question is how to handle cause after cause. I think it's possible to place "invalid" lines into quickfix, so those could serve as headings.

Waiting on clojure-emacs/cider-nrepl#451 to push up the vimscript corresponding to that PR.
However this works, really well.
