Thanks for the request.
I have some heuristics in the context of the LaTeX physics package that can deal with this. However, they are not yet released "into the wild". I've only just included some of the bra-ket notation heuristics.
I might consider including them in the next release.

I once made an extension to specify custom speech https://github.com/krautzource/mathjax-aria-label/ (I haven't had time to updated it to MathJax v4 yet though).
