Issues Copying/Download/Importing any media to system with SD2Vita

Issue
I cannot import any form of media to my Vita, both music files like mp3's and video files like mp4's. I've tried to:

Import media to my system with Qcma (generic "Could not copy some content items." error.)

Export media with VitaShell (Video I receive error 0x8010540E music I receive error 0x8010530B)

Import media with Media Importer (shows up in database but says it cannot be played)

Downloading mp4 video which plays in a browser (file will download but not import; Could not install C2-11934-8)
and they all failed and gave error messages
I tested importing media to the internal storage on my Vita while running my CFW, and all the methods above worked, but as soon as I mounted my SD2Vita to ux0 the errors returned.
What I can do with Qcma is import pictures and games, I am also able to export pictures and games. And I am able to download pictures from the internet.
Any help trying to regain system functionality without resorting to using the original memory card would be great!
Quick update, re-tested my steps but in FW 3.68 and it still had issues.
- Which SD2VITA version do you use?
- Did you try using a different micros card? SanDisk cards are very unreliable, in my experience.
As I said in my post SD2VITA Micro SD Adapter V3.0, it's black with red text on it.
I've also tested it with my Samsung Micro SD Card, same issue
