Issue Summary

The content of notes and dairy form are not resetting once submitted. This creates a problem of user have to manually clearing the form fields to submit new entry.

Steps to Reproduce

  1. create and submit an entry of notes or dairy

Actual Results

Expected Results

  • The form should be automatically cleared once entry is submitted
