Store address fields are hidden when the Store Phone field is hidden

The store address fields do not appear on the Store settings tab in the WC Vendors Pro dashboard when the Store Phone field is hidden under WC Vendors > Settings > Form > Settings.

I think we are good here! I even installed it on the living books mp just to make sure, and it looks like it's working as expected!
