Custom location of .markdown-doctest-setup.js

Currently there's no other way to use a custom- name/location of the config file. If you already have a large amount of config files it'd nice to have a option like --config <path> where you can specify a custom config-module.

Good idea. Do you want to make a pull request?

Still maintained but not an active development priority for me, so I'm happy to merge pull requests but unlikely to make new ones ;)
