Support single changelog and readme

Currently a .rst and .md file exists for each of the changelog and readme. The md type is preferred for ease of writing/viewing, but the .rst is currently used for sphinx. Look for a solution to either let sphinx directly use the .md or automatically generate the .rst from the .md.
