Handle issue expansions where the title contains other issue expansions

If a title of an issue contains "GH-100", when we expand GH-32 to [foobar GH-100 #32](https://lorem.ipsum/dolor/sit/amet/#2, we shouldn't expand the GH-100. Similarly, for any of the other expansions.
